I am 70 years old and in excellent shape.  I sometimes hike 30 miles  a day.  I met Dr Commons while hiking with a outdoor group on Mt Shasta. My hip flank area above my buttocks made it impossible for me to look good in most clothing items.  I was a slender marathon runner but had this deposit of fat around my middle section.  Dr Commons explained it was a lidysproportion and could not be dieted away any more than a large nose or dumbo ears.  I had a very simple liposuction at Dr Commons facility.  It was a breeze.  I was back to work in 3 days and never needed more than Advil for pain.  I felt safe and cared for by Dr Commons and his marvelous staff. LOWER EYELID BAGS AFTER JUVEDERM. WHY? HYALURONIDASE ALWAYS WORKS!

I firmly believe lower eyelid injections with Juvederm are excellent but must be done on a serial basis.   I have inadvertantly become a local referral center for hyluronidase treatment after Juvederm or Restylane mishaps.  I bought the kit for one of my patients so I was the local guy with the hyalronidase.   The biggest problems are the patients that had large amounts injected at on setting.  This is not ideal.  Do  2 or 3 small treatments and this will not happen.  Once it does happen, I have found the following will work.   Stick with the hyluronidase. It will work.  If the batch your doctor is using is not working, ask the doctor to order new and current hyaluronidase.  It must be kept refridgerated so if left out it is useless.   It must be in date.  I am convinced there are simply weak batches of hyluronidase.  Hyaluronidase is expensive and must be ordered in 12 vial lots.  Each vial must be used once and tossed.  I have found in resistant cases that simply using a little more and a little stronger usually does it.  In summary……Hyaluronidase will work but it must be a new batch that has been well treated and refridgerated.  Like fine wine, if mistreated it is useless.   DO NOT DO SURGERY.  YOU WILL BE MUCH WORSE.  I HAVE SEEN THIS HAPPEN ALSO.     Hyaluronidase always wouks if done PROPERLY.  Keep the faith but insist on the above. Because it is costly some may be tempted to use outdated stuff.  Cant do that.  As wonderful as hyluronidase is it is a very very fussy product that demands much love and care and attention to guidelines.   Also check your pre op photos to be sure you are perceiving  things correctly.  Live Forward.  Dr Commons

Exercise after a face lift. When? How much? How Rigorous?

I am very liberal regardina facelift and exercise.  I ask patients to walk but don’t run for the first two weeks.  No gym for two weeks.  No straining for two weeks.  Just gentle progressive normal walking.   At two weeks I have allowed patients to go for it.  I tell them it may hurt to do it but THEY CANNOT AND WILL NOT HURT THEMSELVES WITH RIGOROUS EXERCISE.  I tell patients to let comfort be their guide after two weeks.  I feel strongly that after two weeks there is virtually zero rrisk of bleeding which is the main concern for the first two weeks.   At two weeks you may sky dive, bungie cord jump, or whatever.  If you swell a little or more so what? That will go away.  Sweating helps get rid of Sodium and will soon aid in swelling reduction.  I know I am quite liberal on this and other surgeries;  all are told to go for it after two full weeks post op.  Now understand that if some problem comes up during the first two weeks,  (infection, wound problems, etc), my advice will take this into account and a more conservative approach will be applied.  I have follow this course for over 25 years and over 1700 + facelifts of all types and I have NEVER regreted getting patients exercising at two weeks.  People haveing cosmetic surgery are not invalids.  They are dynamic active people and they WANT TO GET ON WITH IT.  ONWARD I say.  My Best,  G Commons

What Can Be Done For Aging Lips and Mouth? A Review! Dr G Commons

Lip and Mouth concerns may be congenitally based or an age acquired problem. If lips are excessively large or asymmetric, they can be reduced or adjusted. If lips are thin and deficient, they can be made fuller. Lip enhancement can be accomplished with fat transplantation which can be of long term or semi permanent durability. Other fillers include hyaluronic fillers such as JuvedermT and Restylane®. Lip reduction is a pure surgical reduction procedure.

Major issues about the mouth and lip area include lines. These lines are somewhat based both on age and heredity. Smoking certainly does not help. The primary causes of lines include fat and collagen loss. The corrective procedures include dermabrasion, chemical peels (see non-surgical section), and fat injections or hyaluronic fillers. It is possible to return lips to a youthful fullness and reduce lining by 90% or more.
